In order to expedite your first visit Progressive Chiropractic please choose the PDF New Patient Packet that meets your situation. The doctor will need the forms filled out as completely as possible as he will use the information to help in your diagnosis and treatment recommendations. Please also note that there are separate body diagrams for EACH area of your body for which you have a complaint. Use each section for an individual area of complaint.
